Comprehensive Collaborative Care

The Wildleaf Collaborative Care model offers patients a holistic, personally tailored care journey. Nurse Cam and Dr. Jon have a strong focus on empowered education and personalised medicine to promote positive health change, delivered through a close knit care team. They believe that certain natural medicines can serve as powerful catalysts for change when paired with mindfulness and intention.

Booking this consultation provides you with an initial nurse education session followed by a same-day initial doctor consultation.

Initial consults: $160 (15 minutes of Doctor delivered care and 25 minutes of Nurse led education)
Follow ups: $80 (20 minutes)

Am I eligible?

You may be eligible for plant-based medicine if:

  • If you have a chronic medical condition that has lasted longer than 3 months
  • You have trialled conventional medical treatments without relief of your symptoms
What will my consultation involve?

Discussing your medical history including any medications you may already be taking (please provide us with a medication list prior to your initial appointment or bring any medications/supplements with you).

Discussing your current lifestyle habits, how these can impact on your overall wellness, and any areas we may be able to assist with. This may include advice on nutrition, physical activity, sleep, and stress management techniques.

Depending on your health needs, some basic tests may be performed, including blood pressure, resting heart rate, weight/height measurements.

Discussing whether plant-based medicine may be suitable for you and if you meet the recommended guidelines for prescription.

Discussing any potential risks and benefits of using plant-based medicine, as well as current driving laws.

Your practitioner will then develop an individualised treatment plan to help you support your endocannabinoid system through a variety of methods (this may include plant-based medicine if eligible).

Please note:

Any questions asked by your medical practitioner are for the purposes of gaining a better understanding of how you are in order to offer care that is tailored to you as an individual. Wildleaf Clinic is a judgment-free space where we desire everybody to feel comfortable to be themselves in their purest form.

We actively encourage Wildleaf patients to keep a journal of their symptoms whilst receiving care. Engaging in a regular practice of self inquiry and reflection can allow for the development of pattern recognition and coping strategies to help you manage your symptoms.

How do I access my scripts?

If you have been prescribed plant-based medicine, your script will be sent to the TGA for approval and you will receive a call when it is ready to collect, usually within 2-3 days (same day service coming soon). You own your script and can take it to any pharmacy to fill, however, for your convenience we are situated right next door to the friendly team at Wildleaf Dispensary. Scripts are valid for 6 months from time of issue.

Can I request specific medicines?

Our practitioners are trained in the complexities of plant-based medicine and take into account many factors when deciding which medication may be appropriate for you and your medical needs. We believe patients should feel empowered and informed about their treatment and at Wildleaf Clinic, we actively engage in shared-decision making with our patients when planning care. We are certainly happy to discuss any preferences you may have but ultimately decisions around prescriptions are based on the professional opinion and sole discretion of your medical practitioner.
